Bioprosthesis are commonly used in the elderly population. Structural valve deterioration affects the long-term durability.

The Authors report an early deterioration of a Mitroflow valve caused by ring deformation and prosthetic leaflet rupture. The 69-years-old patient, treated at the Centro Cardiologico Monzino, underwent successful redo surgery with excision of the bioprosthesis and placement of a mechanical valve.
